• Copy of Photo ID (Drivers License) 

• Copy of Social Security or Medicare Card

• Homeowners Insurance (copy of declaration page only)

• If you live in a Condo or Townhome – HOA contacts

• Any existing loan information (Lender, etc)

• Third party contact information. (Name, Address, Phone)

• Payment for Appraisal

• Original Counseling Certificate (may be submitted when counseling is completed)
